Bar/Bat Mitzvah Service Information

Dear Parents:

In order to help us prepare for your child's Bar/Bat Mitzvah, we ask that you please try to complete this form three weeks before your child's Simcha. Please specify professional titles such as Dr. or Judge where applicable.  You will be contacted shortly regarding the time and date for the FINAL REHEARSAL.

Please list names and ages of all of Child's siblings.
Only ONE gift is presented by all siblings, either a KIDDUSH CUP, CANDLESTICKS, MEZUZAH, YAD, MENORAH or SHOFAR. If your choice of gift is something other than the items listed, please check with the Clergy for final approval. The Talit will be placed on the child's shoulders by all the grandparents present. If there are no siblings or grandparents, appropriate substitutions can be made in consultation with the Clergy.
When assigning Aliyot please list their names and their relationship to the Bar/Bat Mitzvah celebrant.
